Job Description

Hospice RN Case Manager – Tucson Hospice

AdvisaCare is seeking a compassionate and experienced Per Diem Hospice RN Case Manager to support patients and families throughout the hospice journey. In this role, you will provide skilled nursing care in the home, coordinate individualized care plans, and help patients receive comfort, dignity, and support when they need it most.

What you’ll do

  • Complete comprehensive nursing assessments and monitor changes in patient condition.
  • Develop and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Provide symptom management, medication education, and skilled nursing interventions.
  • Educate and support patients, families, and caregivers throughout the hospice experience.
  • Coordinate care with physicians, social workers, chaplains, aides, and other clinical partners.
  • Participate in interdisciplinary team meetings and communicate important changes in patient status.
  • Complete accurate and timely documentation in the electronic medical record.
  • Follow hospice regulations, clinical standards, and AdvisaCare policies.

Requirements

What we’re looking for

  • Current, unrestricted RN license in Arizona. Compact Licenses Accepted
  • At least one year of hospice, palliative-care, home-health, or related nursing experience preferred.
  • Nurses with strong critical-care, emergency, or med-surg experience may also be considered.
  • Strong clinical assessment, organization, communication, and critical-thinking skills.
  • Comfort working independently in patients’ homes and coordinating care across a clinical team.
  • Reliable transportation and the ability to travel throughout the assigned service area.
  • Experience with electronic medical records preferred.
