Job Description
Purpose
The Contract Licensed Mental Health Professional (LMHP) provides clinical behavioral health services to clients receiving services through Divine Intervention Rehabilitation, LLC. The LMHP is responsible for completing clinical assessments, reassessments, treatment planning, clinical documentation, and other authorized behavioral health services within the scope of the professional’s Louisiana license.
The Contract LMHP is expected to provide clinically appropriate, person-centered services and maintain documentation in accordance with organizational policies and applicable Louisiana Medicaid, Louisiana Department of Health (LDH), Managed Care Organization (MCO), and professional licensing requirements.
Qualifications
The Contract LMHP must:
- Hold a Master’s Degree in Social Work, Counseling, Psychology, or another qualifying mental health-related field from an accredited institution.
- Hold a current and unrestricted license to practice in the State of Louisiana through the applicable professional licensing board.
- Licensed Clinical Social Worker (LCSW) and/or Licensed Professional Counselor (LPC) preferred.
- Have a minimum of five (5) years of direct clinical practice experience preferred.
- Have experience providing behavioral health and/or community-based mental health services.
- Maintain all continuing education and professional requirements associated with their license.
- Complete and maintain LOCUS and CALOCUS certification as required by Divine Intervention Rehabilitation, LLC.
- Successfully complete all applicable background screening requirements.
- Successfully complete required drug screening.
- Maintain professional liability insurance and any other credentials or documentation required for independent contractors.
General Responsibilities
The Contract LMHP is responsible for providing clinical services, including, but not limited to:
- Conduct initial clinical assessments and reassessments.
- Compile and review clinical information necessary to complete assessments and reassessments.
- Conduct required interviews with clients and, when clinically appropriate, family members, caregivers, and/or significant others.
- Establish appropriate DSM-5-TR or successor diagnoses and applicable ICD-10-CM diagnostic codes within the scope of professional licensure.
- Complete LOCUS and/or CALOCUS assessments when required to assist with determining appropriate level of care and medical necessity.
- Develop, review, and update Individualized Service Recovery Plans (ISRPs) in collaboration with the client and treatment team.
- Provide individual counseling/psychotherapy and other authorized clinical services within the scope of professional licensure and credentialing.
- Counsel and communicate with clients, families, caregivers, and appropriate community stakeholders regarding treatment needs and the facilitation of the client's treatment plan.
- Monitor client progress and make clinically appropriate recommendations regarding treatment goals, interventions, services, and level of care.
- Develop transition and discharge plans in conjunction with the client, family/caregiver when appropriate, and multidisciplinary treatment team.
- Complete progress notes and other required clinical documentation accurately and within established documentation timelines.
- Maintain accurate, complete, and timely clinical records in Lauris Online.
- Ensure Clinical Assessments, Reassessments, Individualized Service Recovery Plans (ISRPs), Transition Plans, Discharge Summaries, and other required clinical documents contain all required signatures.
- Provide crisis intervention services when clinically appropriate and within the scope of professional licensure.
- Notify the appropriate medical and/or clinical provider of significant changes in a client's physical or mental health status.
- Complete and/or assist with prior authorization and continued-stay authorization requests for Healthy Louisiana members and other applicable payers as assigned.
- Provide authorized behavioral health core services within the scope of professional licensure and organizational credentialing, excluding medication management unless independently qualified and authorized to provide such services.
- Participate in treatment team meetings, case consultations, and coordination of care when required.
- Cooperate with clinical record reviews, utilization reviews, quality assurance activities, audits, and accreditation or regulatory reviews related to services provided by the contractor.
- Maintain client confidentiality and comply with HIPAA and all applicable federal and state privacy requirements.
- Maintain knowledge of current behavioral health practices, evidence-based interventions, professional standards, and applicable requirements.
- Maintain all required licenses, certifications, credentials, continuing education, and professional documentation throughout the contractual relationship.
- Perform other clinically appropriate contracted services as mutually agreed upon and within the LMHP's professional scope of practice.
Documentation Requirements
The Contract LMHP is responsible for ensuring that all documentation associated with services provided is accurate, complete, and submitted according to Divine Intervention Rehabilitation, LLC requirements.
Clinical documentation should be completed within 24 hours of service and no later than 48 hours, when permitted by applicable requirements and organizational policy.
Documentation must support the service provided, medical necessity, treatment goals, interventions, client response, progress, and continued need for services when applicable.
